2. First name field
3. Phone number field
4. Subscription consent disclaimer “By entering your phone number you are subscribing to receive mobile alerts and donation solicitations from [campaign name].“
   
5. Messaging disclaimer “Message and data rates may apply. Frequency varies. Text HELP for info. Text STOP to [your short code] to cancel.”‍
